UPDATE 4/17/2025: The Rock Island County Regional Office of Education is moving to a new location. We will be moving the last week in April and our offices will be closed during this move (April 28 - May 1, 2025). There will be no fingerprinting services available during those 3 days. On May 1, 2025 we will be back open at our new location of 3561 60th Street, Suite 310, Moline, IL 61265.

Hours: Monday - Friday 8 AM - NOON and then 12:45 - 4 PM

You will need your fingerprint form and either a Drivers License or State ID when you come in. Please be sure your ID has not expired. Expired IDs are no longer excepted, and you will be turned away.

These forms are for the name intended only.  Your employer should provide you with a form before coming to this office. If you are a School District, Contractor or Health Care worker you will need to get your form from your employer, the forms below will not work for those entities. If you do not have the proper form you will be turned away until you come in with one.

Fee Applicant Consent Release  (this form is for the entities indicated on the form only there is a $63 fee for any of these)

Substitute Teacher Fingerprint Form (this form is for a substitute teacher only there is a $63 applicant fee you will pay)

UCIA-Applicant Fingerprint Form (this is for those that are being fingerprinted for IL State only!  There is a $32 fee for this)

We accept debit or credit cards for payment, but for these forms of payment a $3.00 fee will be added.  Cash is accepted or a company check.  NO personal checks will be accepted!
